Students with visual, hearing, medical, psychiatric, cognitive and physical disabilities

Students with visual, hearing, medical, psychiatric and physical disabilities are required to provide a completed CSD medical certificate dated within the last 12 months from the appropriate medical professional e.g. audiologist, neurologist.

Students who are unable to provide a CSD medical certificate or who already have documentation should ensure that the information requested on the CSD medical certificate is included in the documentation submitted.

It is preferred that documentation be dated within the last 12 months so that we have current information regarding a student's disability needs in the academic program.

Depending on the nature of their disability and the length of the program, students may be asked to provide updated medical documentation to the CSD throughout the duration of their program.

Students with temporary medical conditions (e.g. broken arm) will be required to provide a CSD medical certificate or other appropriate documentation.
