Medical statement and deferred final exam

If you miss a final exam because of incapacitating illness, you may apply for a deferred exam. You must complete the Application for Deferred Exam and the Medical Statement form. The Medical Statement form must be completed, signed and dated by the treating physician no later than 24 hours after the scheduled examination. Both forms must be submitted together to the appropriate faculty office within five working days of the missed exam.   The Medical Statement form may also be used as a supplement to various appeal requests. This form must accompany the appeal form it supports (fee, late drop, academic standing, etc.) and submitted to the appropriate office as indicated on the form.

Alternatively, you may seek to defer a final exam for religious observance by completing an Application for Deferred Final Examinations for Religious Observances form. This form must be submitted to the faculty office no later than three weeks prior to the first day of the examination period.
